1. Fill out the "Case Preparation Request" Form.
2. Fax Case Preparation Request to (949)612-1846 or email it to _________________. Along with Credit report and Credit Counseling Certificate (if you are not having us facilitate these documents).
3. We will then contact client and request paystub/income information as well as give them directions on how to complete our client questionnaire.
